Northland Counseling Center Inc. has a team of case managers that provide individual adult mental health case management services. In order to qualify for case management you must complete a diagnostic assessment, and have a diagnosis of a Serious and Persistent Mental Illness, (SPMI).
How can Case Management help you?
Case Management services are activities that are designed to help adults with serious and persistent mental illness in gaining access to needed medical, social, educational, vocational, and other necessary services as they relate to the client’s mental health needs. Case management services include developing a functional assessment, and individual community support plan, referring and assisting the person to obtain needed mental health and other services, ensuring coordination of services, and monitoring the delivery of services.
How can I learn more about getting Case Management?
Referrals for case management can be made by your therapist, medication prescriber or other social service agency, referrals are submitted to Itasca County Health and Human Services for approval. You can also contact Karolyn Teeters at Northland Counseling Center Inc. for more information.
