If you would like to have access to Northland’s Client Portal for yourself or child(ren) please fill out the appropriate form below. Client portal will allow you to be able to fill out forms and check out when your next appointment is.
We are in a process of developing our Client Portal and continually working on features and updates to the Client Portal.
If you are over the age of 18 and would like to request access to Client Portal for yourself click here. If you are 18 and over and have a guardian or under the age of 18, please choose the proxy form below to request access.
If you have a minor under the age of 18 OR are requesting a proxy account for someone over the age 18, who you have guardianship over, please submit the appropriate proxy form below:
Minor Proxy Form Ages 0-11 – This form does NOT require signature from minor.
Minor Proxy Form Ages 12-17 – This form requires signature from minor.
Proxy form for Ages 18 and up – this form requires a signature from both parties.
