Thank you for choosing Northland! Northland is now offering you the flexibility to submit most of your forms online. Once you have completed your forms, an intake specialist will be contacting you.

Online Intake packets can be found below; these forms are completed at intake and annually thereafter.  You also can print the forms and mail them if you prefer (forms below).  If you would like to print, fax or email the forms you can print the below forms or you can request the forms by contacting us at 218-326-1274.

Northland offers individuals and families who are uninsured and/or under insured the option to apply for a sliding fee scale.  If you would like more information,
