Online Intake packets can be found below, these forms are completed at intake and annually thereafter:
- Ages 18 and over NEW client intake packet
- Ages 18 and over EXISTING client update packet, please use this if you are a current client of Northland.
- Ages 11-17 intake packet
- Ages 0-10 intake packet
