Thank you for choosing Northland Counseling Center, Inc.! You can complete your intake online for your teen! Please take a few moments to complete the following forms and your teen will need to complete one as well:
- Minor Registration Form Ages 11-17
- Ages 11-17 Health Questionnaire (HQ)
- Minor Authorization Ages 11-17
- SDQ – Caregiver Form
- 11-17 Screeners (Child Does)
- Release of information (one agency per form – we like to have primary clinic and past mental health agencies)
- Provider Supervision Form
