- or go to your local emergency room.
In order to schedule you for an initial appointment we need to collect the following paperwork. Annually, thereafter; we will ask that you fill out the paperwork in order to update our records.
If, at any time, you have questions on the paperwork or want to know why the information is needed please feel free to call 218-326-1274 and ask to speak to one of our intake Staff.

If you do not receive a call from a NCC staff member within 2 business days of submitting your paperwork, please contact your local Northland Counseling Center, Inc. location to ensure they have received your paperwork.
ONLINE FORMS:
New and current clientele will complete the below form packets:
- New clientele will complete the NEW ADULT INTAKE form packet.
- Current clientele will complete the ANNUAL ADULT UPDATE form packet.
Forms are completed through DocuSign. You will need to enter Your First Name, Last Name and Email Address*. You will complete the forms in the packet and submit when finished. Once, received one of our Intake Staff will contact you.
*If you do NOT have an email address, contact our Intake Staff and they will assist you with obtaining a form packet, or please print the form packet that is located below.
If you have a Guardian and/or Power of Attorney, please contact our Intake Staff prior to filling out the forms.
